Our Central Fabrication accreditation program is geared towards businesses that custom fabricate orthotic, prosthetic and/or pedorthic items/devices for external patient care providers.
The application approval process takes up to two weeks and we will notify you as soon as your facility has been approved for an onsite survey. Your survey can occur as soon as 24 hours after your application has been processed. All first-time applicants will be surveyed within 90 days of application approval.
Your accreditation is valid for up to five years. To maintain your accreditation, you will need to apply for renewal and be resurveyed.
