ABC will consider all written reinstatement requests. All reinstatement requests must include the following information: (i) the date of the final Committee or Board disposition; (ii) a complete statement of reasons that the ABC Credential Holder believes support the reinstatement request; (iii) proof that the ABC Credential Holder has complied with requirements associated with the Committee or Board’s actions and (iv) copies of all relevant documents and materials supporting the reinstatement request. ABC will provide its written decision to the ABC Credential Holder within 45 days of receipt of the reinstatement request and all requested information.
