You will be required to submit a copy of your high school diploma, GED certificate, and/ or official transcripts with your application. If you are extending your credential, your previous file will be referenced if you apply within seven years following the expiration of your original eligibility period. These documents must be uploaded to the online application dashboard or can be faxed or emailed directly to ABC.
If you are applying under Pathway 2, you will be required to submit the Orthotic Technician Competency Attestation Form and/or Prosthetic Technician Competency Attestation Form, verifying that you have obtained the necessary knowledge and skills needed to perform the functions of an orthotic and/or prosthetic technician.
The experience required to become
ABC certified as a technician is documented
on the attestation form. If you do not
possess a particular knowledge or skill, the
attester/supervisor should work with you
prior to completing the attestation form.
Your supervisor will attest to your place of
employment, length of employment, hours
worked and level of practice.
