Mailing List Request Form

ABC mailing lists are at a cost of $0.30 per individual name and $0.60 per facility, are for one time use only, and include names, designations and addresses (does not include email, phone or fax numbers). We ask that you provide us with a brief paragraph describing your use for the mailing list or provide an electronic version of the mailer you plan to send. The list will be provided as an Excel spreadsheet and made available upon receipt of your signed Data Use Agreement and payment. ​
